Home Safety Assessment

$99 price may vary based on location

The RN visits the patient and family in their home.

 Conduct a full safety evaluation:

  • Identify fall risks

  • Recommend home modifications

  • Review optimal furniture placement and mobility pathways

  • Provide expert advice about durable medical equipment (DME) for recovery

Comprehensive Health History

$99

The RN completes a holistic health assessment, including:

  • Past medical history

  • Current medications

  • Allergies

  • Mobility baseline

  • Nutrition, hydration, and sleep habits

  • Emotional and mental health

  • Social support and family dynamics

Individualized Care Plan

A summary of the surgical procedure and expected recovery trajectory

  • Anticipate pain‑management needs

  • Medication timing and safety considerations

  • Mobility expectations and fall‑risk precautions

  • Wound‑care treatments

  • Nutrition, hydration, and rest recommendations

After Surgery

We will provide expert care, teaching, and structure for success.

Allow your loved ones to be present, with flexibility to maintain their regular schedule.

Receive an informed and updated care plan prior to the RN’s completion of care.

Surgery Center to Home

$60 per hour

At the surgery center, the RN:

  • Attends the discharge education session

  • Clarifies instructions directly with the surgical team

  • Advocates for the patient if they are groggy or overwhelmed

  • Assists with safe transfer to the vehicle

  • Transports the patient into the home safely

One to One Expert Care

$60 per hour for up to 48 hours

At home the RN provides:

  • Nonpharmacologic pain management and comfort

  • Medication Regimen, Setup and Training

  • Activities of daily living (ADL), assistance and training

  • Clinical monitoring

  • Emotional and educational support

  • Modifications to the individualized care plan

Opt-In to Ongoing Care

$50 per hour

The RN can continue support through:

  • phone or virtual visits to answer questions and provide guidance

  • on-going monitoring through additional in-home visits

  • transport to follow up appointments with the surgery team

  • advocate for needs at post-operative appointments
